What “immediate implant” actually means

Two different things are often mixed up, and it helps to separate them before your consultation:

  • Immediate placement — the implant is inserted into the extraction socket during the same visit as the tooth removal.
  • Immediate loading (“teeth in a day”) — a temporary tooth is fixed onto the implant shortly after placement.

One does not automatically include the other. Many patients receive immediate placement with a delayed crown, so the implant can integrate with the bone undisturbed. Whether a same-day temporary tooth is safe depends on how firmly the implant seats in your bone and where the tooth sits in your bite.

When immediate placement is considered

The most common situations where we assess a tooth for extraction-plus-immediate-implant in one visit:

  • A tooth broken or fractured beyond repair
  • Severe decay that has destroyed too much tooth structure
  • A failed root canal-treated tooth that cannot be predictably retreated
  • A loose tooth from localized bone loss, where the site is otherwise healthy

Failed Root Canal to Immediate Dental Implant

A root canal-treated tooth can sometimes fail because of recurrent infection, root fracture, inadequate remaining tooth structure, persistent inflammation or progressive bone loss. When retreatment, apicoectomy or restorative treatment cannot predictably save the tooth, extraction may become the safest long-term option.

In carefully selected cases, the failed tooth can be removed and a dental implant placed during the same appointment. This is known as immediate dental implant placement.

Immediate placement does not mean that every infected or fractured tooth can receive an implant immediately. The decision depends on the condition of the surrounding bone, the extent of infection, gum health, implant stability and the position of neighbouring anatomical structures.

Treatment results vary between patients. These X-rays are intended to explain the clinical process and do not guarantee that immediate implant placement will be suitable in every case.

Why Can a Root Canal-Treated Tooth Fail?

A previously treated tooth may require further assessment when there is:

  • Persistent pain while biting or chewing
  • Recurrent swelling or a gum boil
  • Infection around the root tip
  • Vertical or horizontal root fracture
  • Severe decay beneath an existing crown
  • Loosened post, core or crown
  • Significant bone loss around the roots
  • Insufficient healthy tooth structure for another restoration

Not every failed root canal tooth needs extraction. Depending on the diagnosis, possible alternatives may include root canal retreatment, apicoectomy, crown replacement or periodontal treatment. Extraction is considered when the tooth cannot be restored with a predictable long-term outcome.

How Immediate Implant Treatment Is Planned

  1. Clinical and radiographic examination. The tooth, gums, bite and neighbouring teeth are examined. A dental X-ray or CBCT scan may be advised to assess infection, available bone and nearby structures such as the maxillary sinus or nerve canal.
  2. Restorability assessment. Our specialists first determine whether the natural tooth can be predictably saved. Implant placement is considered only when retreatment or reconstruction is unlikely to provide a stable result.
  3. Atraumatic tooth extraction. The failed tooth is removed carefully to preserve as much surrounding bone and gum tissue as possible.
  4. Infection removal and socket cleaning. Inflamed or infected tissue is thoroughly removed. The extraction socket is inspected before deciding whether immediate placement remains clinically appropriate.
  5. Implant placement. The implant is positioned in available healthy bone beyond or around the extraction socket. Good primary stability is essential.
  6. Bone grafting when required. A small gap may remain between the implant and the socket wall. Bone-grafting material may be recommended to support healing and preserve the contour of the site.
  7. Temporary tooth and final crown. Depending on implant stability and the location of the tooth, a temporary tooth may be provided. The final crown is generally placed after the implant has integrated with the bone.

The fastest option is not automatically the safest option. The treatment timeline should be selected according to biological conditions rather than convenience alone.

Who May Be Suitable for an Immediate Implant?

Immediate implant placement may be considered when:

  • The failed tooth cannot be predictably restored
  • Adequate healthy bone is available
  • The implant can achieve good primary stability
  • Infection can be thoroughly removed
  • The gum condition is favourable
  • The patient can maintain good oral hygiene
  • Smoking and uncontrolled medical risks are absent or appropriately managed

When Might Immediate Placement Not Be Recommended?

A delayed approach may provide a more predictable result when there is:

  • Extensive infection or pus
  • Severe loss of the socket walls
  • Insufficient bone for stable implant placement
  • Active uncontrolled gum disease
  • Poor oral hygiene
  • Uncontrolled diabetes or other relevant medical concerns
  • Heavy smoking
  • Inability to avoid pressure on the implant during healing

In these situations, extraction, infection management and bone grafting may be completed first, followed by implant placement after healing.

Can an implant be placed immediately after removing a failed root canal tooth?

Yes, in selected cases. Immediate placement depends on infection control, available bone, socket integrity and whether the implant can achieve adequate primary stability.

Can an implant be placed when there is infection around the root?

A limited infection does not automatically rule out immediate placement, but the infected tissue must be thoroughly removed and sufficient healthy bone must remain. Extensive infection or bone destruction may require delayed placement.

Is root canal retreatment better than an implant?

Saving the natural tooth is generally considered first when retreatment can provide a predictable result. An implant may be considered when the tooth is fractured, severely decayed, structurally non-restorable or associated with persistent disease.

Will I receive a tooth immediately after implant placement?

A temporary tooth may be possible, but immediate loading is different from immediate implant placement. The temporary restoration must not place damaging pressure on the healing implant.
